If you are using a Thermomix® TM6 measuring cup with your Thermomix® TM5:
For cooking (simmering) at temperatures of 95°C or above (200°F), the simmering basket should always be used instead of the TM6 measuring cup, as the TM6 measuring cup fits tightly in the lid. The simmering basket rests loosely on top, is steam-permeable, and also prevents food spatter from the mixing bowl.

Recipe's preparation
Put onion, garlic and whole mushrooms in TM bowl and chop 3 second/speed 5 MC on. Check and chop again if required
Add cubed butter to TM bowl and sauté 5 minutes/temp 100/"Gentle stir setting" "Counter-clockwise operation" MC on
Add water, veg stock and milk to TM bowl. Cook 20 minutes/temp 100/"Gentle stir setting" "Counter-clockwise operation" MC on
Add salt and pepper to taste, sever with crusty warm bread rolls

I like it the way it is but you could add some cornflour if you wanted it thicker
